FDA Approves New Juvederm Vollure XC Filler

Americans looking to turn back the clock to a more youthful appearance have yet another tool in their arsenal as of this month. The U.S. Food and Drug Administration has approved a new hyaluronic acid dermal filler manufactured by Allergan. Juvederm Vollure XC has been approved in Europe since 2013 (under the name Juvederm Volift), but will now be available to Americans over 21. NYC plastic surgeon Dr. B. Aviva Preminger, MD says there are some very exciting benefits with the new dermal filler.

What is Juvederm Vollure XC?

The new dermal filler is approved for “the correction of moderate to severe facial wrinkles and folds, such as nasolabial folds.” Clinical trials found that 59 percent of patients had significant improvements in smile lines around the mouth for up to 18 months. Satisfaction rates were good – with 82 percent of patients pleased with their results at six months, and 68 percent still satisfied at eight.

Juvederm Vollure XC benefits

What makes this particular product so exciting is that it requires fewer injection sessions and lasts up to 18 months. Vycross technology is used to link high and low molecular weight hyaluronic acid together through cross-linking to form a more connected gel. The product is substantial enough to fill depressions, but is soft enough to move naturally with facial expressions.

Does Kybella Live Up To The Hype?

kybella-bna-panelThe U.S. Food and Drug Administration approved Kybella for non-surgical treatment of double chins in April 2015. Since its debut on the market, Kybella has been called “a game-changer,” a “miracle drug,” and a “double-chin killer.” Khloe Kardashian has agreed to be the “face of Kybella” in the new “Chin Up” marketing campaign. Currently, 175 independent reviewers have given Kybella an 83% “Worth It” rating.

Manufacturer Allergan reported net revenues from Kybella sales of $12.7 million in the second quarter of 2016, with representatives anticipating it will one day rival the $2 billion they haul in from another wildly successful product they sell – Botox. “We own the face,” said Philippe Schaison, president of Allergan’s U.S. medical business, referring to their “liquid facelift” combination of Botox, Kybella, and dermal fillers. He said of Kybella specifically: “It’s a disruptive technology. It’s working great, and there’s nothing like it.”

Can you really get rid of your double chin without surgery?

Kybella uses deoxycholic acid to break down fat cells, which are naturally disposed of by the body. The injections are indicated for isolated submental fat deposits without skin excess. The ideal patient is relatively young, with small pockets of fat. Plastic surgery remains the only way to remove loose, lax, excess skin. If you are overweight or have a good amount of excess fat and volume in areas other than just the “double chin”, then your success with Kybella may be limited.

What studies prove the efficacy of Kybella?

The FDA approved Kybella for double chin treatment based on evidence from two clinical trials at 70 sites in the U.S. and Canada that enrolled a total of 1,022 adults with “moderate to severe amounts of submental fat located beneath the chin or lower jaw.” Kybella appeared to work equally well among men and women. The majority of the patients were between the ages of 19-65 and were predominantly white, so differences in response could not be determined.

The results demonstrated that patients receiving up to six treatments of Kybella showed significant reductions in submental fat compared to patients receiving placebos. Nearly 70% of subjects responded to Kybella based on a composite of physician and patient measurements. Patients reported improvements in self-esteem and happiness following treatment with Kybella.

Are there any side effects?

Rare side effects identified during the clinical trials included nerve injury causing uneven smile or facial muscle weakness, and difficulty swallowing. More common side effects included: swelling, bruising, pain, numbness, redness, and areas of hardness in the treatment area. Most adverse side effects resolve on their own within two to four days.

Is Kybella better than liposuction?

Most plastic surgeons will tell you that Kybella is not necessarily better at removing fat than liposuction… but it is considerably less invasive. Patients like that there are no incisions necessary with Kybella injections. This decreases the risk of infection, adverse bleeding, and adverse reaction to anesthesia. There is also less downtime. On the downside, up to 50 injections can be administered in one Kybella treatment session. Most patients require 2-4 treatments, spaced a month apart, to get the results they desire.

Is Kybella worth it?

Kybella is not the first blockbuster drug to debut with so much hype. Other fat removal systems — Liposonix (ultrasound), Vanquish (radiofrequency), Cellulaze (laser), and CoolSculpting (cryo)—have all started out with high approval ratings that waned over the years. On the other hand, treatments like Botox and liposuction continue to enjoy great market success.

How Does Age Perception Impact Facial Plastic Surgery?

A recent study shows that some plastic surgeons have the ability to accurately estimate a patient’s age by analyzing only a photograph. The study is noted for addressing “an issue that is important to facial plastic surgeons and reveals how the results of rejuvenation procedures can be assessed.”

Plastic surgery study prompted by lack of scientific tools

The JAMA Network wanted to take a close look at how facial plastic surgeons evaluate patients to determine possible aesthetic improvements. Subjective elements such as beauty, degree of rejuvenation and perceived age were considered. These are critical components in determining what kind of cosmetic procedures will be recommended to a patient.

This study provides a more scientifically rigorous overview of how plastic surgeons view patients and treatment. While there are numerous treatment options that have proven to be safe, reliable and effective, there remains a question of how individual perception effects ultimate results when it comes to cosmetic procedures.

Results of the plastic surgery study

A group of facial plastic surgeons (7 in total participated in the study) were given 70 photographs of actual patients and were required to write down the perceived age of each patient. Researchers discovered that the average estimates from any 3 plastic surgeons were within 10 months of the given subject’s chronological age. Finding a consistent correlation between the age estimates of a plastic surgeon and a patient’s actual age will provide a firm statistical foundation for future scientific research into plastic surgery outcomes, including patient satisfaction rates.

Eyes are the Window to the Soul

Adult woman with beautiful faceYou are probably familiar with the old adage “the eyes are the window to the soul.”  It is said that the eyes convey a person’s intentions, emotions and inner spirit to others, but they can also communicate a lifetime of sun worship, heavy smoking and even drinking. Because the eyes are one of your first features to show signs of aging, they convey a good deal about your lifestyle choices – both good and bad. The skin around the eyes is incredibly delicate and even more prone to wrinkling than other facial areas. As we age, the fragile skin on the upper and lower lids begins to lose elasticity, giving one a perpetually tired appearance.

In truth, puffy or tired looking eyes is a common complaint among middle-aged men and women, who often turn to plastic surgery to restore a more youthful-looking appearance.  Even the most skilled of make-up artists can do little to camouflage loose crêpey skin on the upper lids and prominent under-eye bags.  Fortunately, there are new techniques in eyelid surgery, also known as blepharoplasty, that provide beautiful, lasting results for patients who want to wake up looking “bright eyed.” 

Upper and lower blepharoplasty

Excess skin (and occasionally fat) in the upper eye lids gives a heavy, aged appearance. If friends or co-workers are constantly asking if you’re sick or haven’t been sleeping well, it may be time to have a closer look at eye rejuvenation. Most of the time, an upper eyelid surgery is purely cosmetic, but in more severe cases, this extra skin can actually impede vision.

Individuals who are bothered by puffy bags under the eyes, or loose skin, may be candidates for a lower blepharoplasty. Surgical techniques may include a transconjunctival approach, in which a tiny incision is placed inside the lower eyelid to remove fatty deposits that create the look of under-eye bags or puffiness.

Alternatively, those who are bothered by extra sagging skin below the eye would fare better with a transcutaneous lower eyelid surgery, where incisions are placed on the outside of the lower eyelid. The majority of patients who undergo lower eyelid surgery opt for general anesthesia, though local anesthesia with sedation is also offered.

Injectables to fill in tear trough area

Some patients may not be good candidates for blepharoplasty, or would rather test the waters with a less invasive procedure. Patients who have dark hollows under the eyes may benefit from injectable fillers, such as Restylane or Juvederm. Unlike the effects of surgery, dermal fillers are temporary and carry a lower risk of complications.

It’s important to note that while dermal fillers add volume, they do nothing to help skin laxity. For this reason, patients with sagging, wrinkled skin and puffy under eye bags are best served with traditional blepharoplasty.

Achieving Lip Fullness with Juvederm

lip augmentationAn exciting new product will soon be joining the Juvederm family. Juvederm Volbella XC has just received FDA approval and is expected to be available to patients as early as October 2016.

Juvederm is already well known for its suite of dermal fillers for the temporary correction of lines and wrinkles of the face. Now, the makers of Juvederm, Allergan, can corner a share of the market in lip augmentation and enhancement.

Juvederm Volbella XC

Juvederm Volbella XC has been approved by the FDA to increase the fullness of the lips and to correct perioral rhytids or perioral lines in patients who are at least 21 years of age. Perioral rhytids are a common problem among smokers, former smokers, and individuals who are older than 50 years. Perioral rhytids are wrinkles that develop around the mouth and are especially evident when a person purses her lips. Additionally, this approval brings to market a product unlike anything that is currently available in the United States.”

Clinical trials of Juvederm Volbella XC

In announcing the approval of this latest dermal filler, Allergan pointed to several clinical studies that the company said establishes the product’s safety and effectiveness. The U.S. pivotal study included 168 study participants who were treated with the new dermal filler. The scientists used a four point scale to evaluate how effective the product was in smoothing out the lines around the mouth. A five point scale was used to evaluate how well Juvederm Volbella XC was able to plump up the lips of the study participants. According to Allergan, two thirds of the study participants demonstrated improvement in both categories through one year after the treatment.

Patient safety is another important consideration when the FDA decides whether to approve a new product for use. According to Allergan, the side effects seen in the clinical trial were similar to those that arose in a control group. These side effects included redness, itching, firmness, swelling, and bruising at the injection site. These issues were temporary and went away on their own. The reported side effects are comparable to the potential side effects associated with other dermal fillers and injectable cosmetic products.

Bethenny Frankel Admits to Having Botox Jaw Reduction

In the past, plastic surgery was the only way to effectively address this issue, but times have changed and Botox has proven to be an excellent solution. Bethanny Frankel, who stars in The Real Housewives of New York, recently had Botox jaw reduction and is thrilled with the results. Selfie Boom Drives Demand for Lip Augmentation

model on white background having botox injectionThere’s no denying that we live in an era influenced by social media and the instant gratification of sharing selfies with the world. But are selfies — which may inadvertently show wrinkles or the beginnings of a sagging jaw line — driving a self-perceived need for plastic surgery? According to a survey conducted by the online community RealSelf, the answer is a resounding yes. And recent data from the American Society for Plastic Surgery (ASPS) backs this up.

The survey asked if social media had influenced RealSelf visitors to consider getting or have some type of cosmetic surgery. Nearly 16 percent answered “yes,” and a third of respondents answered, “somewhat, I knew I wanted a change, but photos on social media made me more aware.”

So what types of cosmetic enhancements are people requesting? 2015 statistics from the ASPS show that lip implants and augmentation have skyrocketed in recent years, making them one of the most popular facial procedures in the U.S.

Lip implant procedures in high demand

In 2015, both male and female patients requested a record number of lip implant procedures, according to ASPS data. Some 27,449 lip implants were performed across the nation last year, which is an increase of almost 50 percent since 2000.

“We live in the age of the selfie, and because we see images of ourselves almost constantly on social media, we’re much more aware of how our lips look,” says ASPS President, Dr. David H. Best Treatments for Laugh Lines

skin check before plastic surgeryLaugh lines are the facial creases that appear around the eyes and mouth as a result of frequent smiling or laughing. Also known as nasolabial folds and crow’s feet, those creases and wrinkles can make you look older than you feel.

Fortunately, there are a number of options for treating laugh lines today, allowing you to choose the procedure that best meets your needs and desired results.

Explaining laugh lines

Laugh lines develop in the outer corners of the eyes due to squinting when you smile or laugh. Also known as crow’s feet, these lines first appear and then disappear as your facial expressions change. Gradually, these lines will become permanently etched into the face, which is when patients frequently look to cosmetic treatments to reduce their appearance.

Other laugh lines, known as nasolabial folds, extend from the corners of the nose to the corners of the mouth. They develop as a result of frequent smiling and, like crow’s feet, can eventually become a permanent fixture on your face. Nasolabial folds can also become quite deep, making them more challenging to treat than other types of facial wrinkles.

Non-surgical injectables

Both crow’s feet and nasolabial folds can be effectively reduced using injectable treatments. Botox injections are frequently recommended for crow’s feet, because they relax the muscles around the eye that lead to the formation of these wrinkles. Botox injections take just a few minutes to complete and require no downtime afterward. Results of Botox are evident soon after treatment and continue for 3-4 months.

Dermal fillers like Juvederm and Radiesse are generally the top choice in treating nasolabial folds. These injectables work underneath the skin’s surface, providing support to the underlying skin structure to smooth away facial creases from the inside out. Dermal fillers are another minimally-invasive “lunch hour” procedure, involve no downtime and typically last many months before a touch-up treatment is required.

Surgical treatments

Patients that are not good candidates for non-surgical treatments or are looking for more permanent solutions to their laugh lines can also consider surgical alternatives. Both facelifts and eyelid surgery can be used to firm and smooth the skin around the eyes, including loose skin that leads to the appearance of fine lines around the outer and lower portion of the eye.

Nasolabial folds can be effectively addressed with a mini facelift that concentrates on lifting and firming skin in the lower portion of the face. In addition to smoothing away nasolabial folds, this procedure will reduce the appearance of other facial wrinkles while restoring youthful definition to the jawline.

Both full facelifts and mini facelifts are performed using general anesthesia and require a recovery period afterward. Mini lifts are usually done on an outpatient basis and involve a recuperation period of a 7-10 days. Full facelifts may be performed as an outpatient procedure, although some patients may require an overnight in the hospital. Recovery from a full facelift usually takes 2-3 weeks. Results of both procedures can be quite dramatic and last up to 10 years for many patients.
